 Please have your insurance card (if applicable) and a payment card ready - these are require to complete the intake paperwork.

​

We require new patients complete our intake paperwork prior to reaching the 24 hour mark before your appointment. If your intake paperwork is not complete 24 hours before your appointment, your appointment will be cancelled and we'll reach out to reschedule. 

​

This policy allows us enough time to adequately prepare for your appointment, for both your provider(s) and our administration. 

​

Our software system sends you an integrated intake form to complete and submit online.

 If you're unable to complete the online intake, please reach out. We will make arrangements for paper documentation to be sent or allowed time to complete in office.

Motor Vehicle Injury 

If you are a current patient or a new patient in a new motor vehicle accident, please select the "New MVA Appointment" to allow for enough time to examine and document your Personal Injury Protection (PIP) case.

Please bring any medical files regarding care for the injuries you have sustained, we can also request them from your providers or imaging centers.If you need additional referrals for other providers we are also able to coordinate care for your specific injuries.

If you have any billing questions please call our office, our front desk staff or billing manager will be able to answer any questions and help you navigate the process.

Cancellation Policy

Cancellation Policy We offer cancellations, at no charge, up to 24 hours before your appointment.

As a patient, you can cancel and reschedule your appointment, online, before the 24-hour cancellation window.

If you cancel during the 24 hours prior to your appointment, a fee will be charged.

Within 24 hours prior to your services, you can reach out to us directly to cancel or reschedule, expect a missed appointment fee to apply. (Subject to our discretion.)

Please note we now require a payment card saved on file to schedule. In the case of a missed appointment or late cancellation, this card will be charged the same day.

Missed Appointment DC/Acu Fees for new patient are much higher than a standard missed appointment fee due to the amount of time reserved with the Provider.

Multiple recurring missed appointments may trigger a dismissal of patient provider relationship.
